Certified Therapeutic Recreation Specialist, CTRS
Activity Consultant Certified, ACC
Experienced Presenter/Educator
Minimum of 5 years long-term care experience

Social Services Consultation

Department and administration (policies, procedures, job descriptions)
Documentation (Assessments/Individualized Care Plan, Social history
Psychosocial Assessments, MDS, RAPs, progress notes, assessment of psychological harm, grievance follow up, Pasrr screening, etc.
Behavioral Management and Psychotropic Mediations (Team treatment approach, ongoing and individualized, mental health support, regulations, care planning for psychotropic medication usage, dose reductions, psychiatric hospitalization

    A one time 8-hour audit (may include plan of correction or written assessment)
    Monthly, quarterly or individualized designed by need
    Specialty In-service training for activity staff, volunteers, residents/family
